Escalate

Re-surface to the out-of-band approver and keep waiting; never self-approve. Default, fail-closed.

Cancel

Terminate the run (fail-safe cancel). Strict mode.

AutoApprove

LEGACY: auto-approve on timeout. The ONLY path to the old fail-open behavior; opt-in only.
