Variants

Requested

A gate opened (a run entered WaitingApproval).

Resolved

A principal resolved the gate (approve/deny).

Escalated

A timeout re-surfaced the gate to the out-of-band approver (fail-closed).

TimedOut

A timeout terminated the gate (fail-closed cancel).
